Player Insurance

Any player suffering an injury during training or matches that may require medical attention must complete an Accident Report Form

The completed form should be sent to the MCC Hockey Administrator, who will enter details from these forms onto a central register. The register details and the forms will be available to players/officials if they later need to make a claim on MCC insurance.

Accident procedures information and an Accident Report Form may be printed via the following links.

The insurance cover is provided through Hockey Victoria and JLT  Sport.
For further information see the National Hockey Insurance Program
It is in the best interests of all players to have Private Health Insurance, and not to rely entirely on MCC’s sports injury insurance cover to relieve financial costs that may occur as a result of injury.
